The landscape of global philanthropy is witnessing a significant shift toward integrated, large-scale service models that bypass traditional fee-for-service structures. At the forefront of this movement is Sri Madhusudan Sai and the One World One Family Mission. Rooted in the ancient Indian philosophy of Vasudhaiva Kutumbakam—the belief that the entire world is one interconnected family—this mission has expanded from a rural enclave in South India to a humanitarian network spanning 100 countries.

As the organization prepares for a high-profile engagement in Atlanta, Georgia, the focus remains on its three core pillars: nutrition, healthcare, and education, all provided entirely free of charge to the underserved.
Nestled in the serene foothills of Nandi Hills in Muddenahalli, Karnataka—about 40 minutes from Bengaluru International Airport—Sathya Sai Grama serves as the vibrant epicenter of this global humanitarian mission. From humble beginnings with a small school in the 1970s, it has grown into a vast educational complex amid natural surroundings. Today, under the leadership of Sri Madhusudan Sai, the campus drives an extraordinary expansion of the foundation’s humanitarian projects, accelerating their scale and worldwide reach.

One of the most pressing challenges across the globe is “hidden hunger” —the chronic deficiency of essential micronutrients. In India, home to the world’s largest child population, many students attend school on an empty stomach, which severely impacts cognitive development and academic retention. To address this, Sri Madhusudan Sai initiated the Annapoorna Morning Nutrition Program in 2015. The scale of this operation is vast. As of 2026, the program serves over 10 million school-going children daily across 150,000 government schools in India. The mission has also exported this model to nations including Malawi, Cambodia, Nigeria, South Africa, Fiji, and Sri Lanka, totaling over 3 billion servings to date.

A critical component of this success is “SaiSure,” a multinutrient supplement developed to meet the specific needs of growing children and expectant mothers. By partnering with various state governments in India—a model described as sarkārā (Government), samājā (Society), and sansthān (NGOs)—the trust ensures that nutrition is not merely a charitable act but a sustainable public health intervention.

The healthcare arm of the mission is perhaps its most technically ambitious. Congenital Heart Disease (CHD) remains the most common birth defect in India, with an incidence of approximately 240,000 per annum. For many families in low-income brackets, the cost of pediatric cardiac surgery is insurmountable, leading to high mortality rates for treatable conditions. In response, the mission established the Sri Sathya Sai Sanjeevani Hospitals. This network has grown into the largest chain of pediatric cardiac hospitals in the world that provides services completely free of charge.

Since the first center opened in 2012 in Atal Nagar, Chhattisgarh, the network has performed over 40,000 successful heart surgeries and interventions. These hospitals operate on a “gift of life” model, where no billing counter exists, and care is provided regardless of the patient’s religion, nationality, or socio-economic status.

Beyond cardiac care, the mission is addressing the shortage of rural medical professionals through the Sri Madhusudan Sai Institute of Medical Sciences and Research (SMSIMSR). As India’s first private rural medical college to offer free education, it aims to produce a new generation of doctors who are committed to service rather than commercial gain. This infrastructure is set for a massive expansion with the upcoming inauguration of a 600-bed free rural teaching hospital in Muddenahalli, which will serve as a multi-specialty hub for thousands of rural patients.

The mission’s approach to education seeks to rectify what Sri Madhusudan Sai describes as a crisis of “information-based knowledge” versus “transformational knowledge.” The Sri Sathya Sai Loka Seva Gurukulam group manages 27 residential campuses across South India and a school in Nigeria, providing values-based education to over 3,700 students from Grade 6 through to doctoral levels. While modern education emphasizes the “4 Cs”—critical thinking, communication, collaboration, and creativity—this mission prioritizes a “5th C”: Character. The goal is to produce graduates who are not only professionally competent but also socially responsible.



To support this, the Sri Sathya Sai University for Human Excellence (SSSUHE) provides entirely free higher education, including stipends for postgraduate students. This ensures that financial barriers do not prevent talented rural youth from becoming leaders in their respective fields.

The year 2025 marked several significant milestones for the mission. In April 2025, Sri Madhusudan Sai was conferred with the “Companion of the Order of Fiji,” the highest honor of the Pacific nation, for his transformative service in the region. He joined Prime Minister Narendra Modi and President Droupadi Murmu as one of the few Indian citizens to receive this distinction. Furthermore, the Universal Peace Council in Israel awarded him the “Highest Peace Leader Award” in August 2025, recognizing his efforts in fostering global harmony through service. The mission also broke new ground in the financial sector; the Prashanthi Balamandira Trust was among the first entities to be listed on the National Stock Exchange’s (NSE) Social Stock Exchange in India, raising funds specifically for emergency and trauma care through transparent social finance mechanisms.

The “One World One Family World Cultural Festival,” a 100-day event held from August to November 2025, further cemented the mission’s global footprint. Recognized by Guinness World Records as the longest cultural festival, it brought together representatives from over 100 countries to celebrate human unity through the arts. This philosophy is rooted in the realization of divinity within oneself and others through selfless service. For the foundation, service is not viewed as a “handout” or “charity” born of pity, but as a “self-offering” based on the recognition of the inherent dignity of every individual.

Inspiring Evening with Global Humanitarian Vision in Atlanta
As the mission looks toward the remainder of 2026, the focus shifts to international community engagement. This global outreach includes an upcoming charity evening in Atlanta, Georgia, which serves as a bridge between the mission’s grassroots work in India and its global support network. The evening is specifically dedicated to supporting the launch and ongoing operations of the new 600-bed multi-specialty Teaching Hospital in Muddenahalli. This state-of-the-art facility is designed to serve thousands of patients in need for generations to come, addressing the reality that millions worldwide still struggle without access to basic healthcare.

The Atlanta event will feature a blend of networking with community leaders, an inspiring talk by Sri Madhusudan Sai, and a formal dinner. It aims to bring the local community together to support initiatives that provide free education, healthcare, and nutrition to underserved populations, helping to create a more equitable future.
